How do you perform it? • Patient lies prone with feet hanging off the edge • Calf muscles are relaxed • Examiner squeezes the calf What should happen? ✅ Normal: The foot automatically plantarflexes (toes point down) ❌ Positive Thompson Test: Little to no movement → suggests a ruptured Achilles What are we looking for? • Loss of force transmission from calf → heel • Asym
